Operationalizing Cardiac ERAS: The 10 Commandments of F4 (Fast Forward to Full Function)
The Fast Forward to Full Function (F4) framework was developed to promote rapid, reliable functional recovery and mobility for cardiac surgery patients across all risk categories. As postoperative immobility remains a major contributor to complications and reduced patient satisfaction, F4 addresses a critical need—especially as less invasive transcatheter options become more widely available and patients increasingly prioritize a faster return to daily life.
F4 is built on the principles of a fully integrated Enhanced Recovery After Surgery (ERAS®) program, centred on restoring mobility with minimal postoperative restrictions. The initiative emphasizes consistent, interdisciplinary implementation to ensure patients regain independence and physical function regardless of the surgical approach.
The authors outline ten core principles (“commandments”) distilled from extensive clinical experience, offering practical guidance for teams aiming to build or strengthen an ERAS program that reliably supports full functional recovery.
