Outcomes of Video-Assisted Thoracoscopic Lobectomy in Octogenarians Following Enhanced Recovery After Surgery Protocols
This large ERAS‑based cohort study evaluated outcomes after VATS lobectomy in octogenarians. Despite slightly longer hospital stay and higher postoperative atrial fibrillation rates, overall complications, readmissions, and mortality were comparable to younger patients. Standardized chest drain management – using a single drain with objective air leak and no fluid thresholds for removal – supported safe recovery. These findings indicate that optimized ERAS protocols, including consistent chest tube management, enable safe surgical treatment in carefully selected elderly patients.
