The Society of Thoracic Surgeons 2026 Clinical Practice Guidelines for the Prevention and Treatment of New-Onset Postoperative Atrial Fibrillation After Cardiac Surgery
The 2026 STS guidelines provide an evidence-based framework for the prevention and management of new-onset postoperative atrial fibrillation (POAF), the most common complication after cardiac surgery. Developed through a multidisciplinary review of available randomized and observational evidence, the guidelines issue recommendations spanning preoperative, intraoperative, and postoperative care. Strongest support is given to perioperative oral amiodarone, perioperative β-blockers, posterior pericardiotomy, and rhythm cardioversion for hemodynamically unstable patients. The document highlights ongoing evidence gaps, emphasizes individualized risk assessment, and aims to standardize POAF management while identifying priorities for future research.
